I give my son/daughter permission to participate in the Kapolei Youth Sports Camps and have no knowledge of any medical conditions that would prevent him/her from participating.  I acknowledge that these activities involve risk and the need for responsible behavior and obedience on his/her part.  I understand that Elam Strength will not be held liable for any injuries incurred while participating in the youth sports camps.  Likewise I also agree to hold harmless those coaches and members covered under the Elam Strength staff.  I give permission for media shots of my child to be used for Elam Strength informational and promotional purposes.  In the event of an accident or injury, I agree to my son/daughter receiving first aid and medical treatment and authorize transportation of my child, by ambulance if necessary, to the nearest available medical facility.
